Bring one of those air riffles for hunting, 20k pellets, and extra gaskets. Us it for any unarmored target, use a real firearm sparingly for intimidation and armored opponents.

Anyway, I'd totally overload myself with gear. A mail shirt and half plate (with modern metallurgy, obviously). A light-ish arming sword, a longsword, a halberd, a dagger, a buckler. Three (3) glock 19 with a bunch of ammo. A single robust bolt action with lots of spare parts and a decent amount of ammo. Clothes, from thick polycotton, thinsulate (more robust than most other fills) and linen. A field knife (stainless, super steels, not too hard to sharpen). A really robust sleeping bag, a bunch of other equipment.
Once I'm there, I'll pretty soon get rid of all the stuff that don't fit the place and my status as a total stranger. Find some weapons training, keep only the weapons that seem to make sense for the context. Firearms are useful for a limited time (ammo gonna run out) so only use in case of emergency. Apart from those, I'm basically limited to really good versions of medieval weapons, and eventually even those will degrade. Gotta establish yourself in the meantime.
Oh, and more thing: A bunch of medicine and simple instructions to use them. Once you prove to someone they work, you'll be swimming in money while it lasts.

Since survival is the main point I focused on sustainability. gold bar to get me started with funds, reloading kit to make my ammo last longer, Tabacco can be cultivated as a cash crop, potatoes to be able to sustain and possibly sell. as for combat I focused on a more passive/recon approach with nv/ir and suppressor could have switch it out for a bolt action but I chose the marlin for being able to use black powder loads.
I kit myself with shittons of science and engineering books and team up with the first wizard I can find.
The medieval world sucks dick, but being able to ignore physics while knowing how physics works when you aren't doing that is gonna make it suck a lot less dick. As long as nobody reads English, I'm fricking invaluable.

Supressed 10/22 and a few bricks of ammo, a .44 mag revolver for that thunder and lightning coming from my hand to explode heads magic, a nice knife, a lighter, a crank charging flashlight, antibiotics.
Wizards will not bother me after they see me explode heads, create fire with a flick of my thumb without even mumbling a spell, and killing 30-50 knights with a whispering stick.
