The book doesn't state, the author specifically didn't say even outside the book cause he said it "doesn't matter" but he had researched big meteor strikes. Only way to get all that ash in the air would be lots of volcanoes going off and a meteor strike would fit that bill. Plus there was that building that was super heated enough to bend and then cooled. Don't think a nuke could do that. You'd think it was a nuke cause that would explain the father's sickness but I guess he was just sick from something else.

Not really, there's just the trapdoor hatch and presumably an air pipe somewhere nearby.
Even if the door could be locked from the inside they could find their air vents, plug them and just wait for them to die from asphyxiation
