Low-flying CAS is optimal because it increases situational awareness and reduces time-to-target. In a MANPADS situation the CAS must fly outside of the threat’s engagement range, or attempt Wild Weasel if training and machines allow. Doing Wild Weasel takes the plane off the CAS mission and flying high decreases withal awareness and increases time-to-target. Helicopters are another option able to hide from threats while providing CAS but position becomes a complicating factor. This is talking about America mind you.
Russians doing CAS rely mostly on dumb bombs and rockets so must fly low; even Soviet-era planes have ballistic computers but there are too many factors which impact accuracy which can only be minimized by flying as low as possible. The pilots don’t have the training to do Wild Weasel so any AD threat means they can’t do CAS. Their choppers can snipe things at max range but they don’t have the tech to do anything more. Ukrainians have JDAM-ERs but this is a kludged-together solution so they’re really only for pre-planned targets— and Ukrainians are only able to manage a handful of sorties a day for whatever reason meaning their troops simply don’t get CAS at all.

We can do CAS from strategic bombers. MANPADS are a threat to specific forms of CAS, but you need actual AA systems to deter CAS in general.
