The US government oversees and upholds a capitalistic system where the people gets a say in where society's resources go, and they heavily prioritize not starving. The government also helps things a long a bit at times (at times directly, at times indirectly), because it knows anyone who went hungry is unlikely to have forgotten that on election day.
The NK system puts most of that power into the hands of the government, which puts far less emphasis on everyone having enough food and far more emphasis on the military. And so many North Koreans go hungry/starve to death, while on the US government's watch they wouldn't have.

Most of the military doesn't work, has no functional equipment and no proper training because their only cause for existence is to kill civilians if they rise up.
Other shithole countries with less stability simply have an elite that is too greedy to do the same.
Because North Korean policy is literally military first, people's livelihoods last. In any other country this kind of policy would have gotten the rulers overthrown but decades of extreme brainwashing in Best Korea did the trick and made the population docile. Plus China and Russia have helped in propping up the regime as they literally share a common border with them.
No one knows what condition this army is actually in. Parades are like make-up, they're just for show.
Chances are their military is in far worse state than Russia's in terms of how well maintained it is. Numbers mean frick all when the troops are malnourished; have little to no training; barely any ammo; and a rifle that doesn't shoot straight.
