Honestly, I'd expect the majority if not all of any nuclear attack to get shot down at this point. If current Patriots can blast incoming hypersonic missiles out of the air a falling warhead really isn't safe from interception. I'm sure we've got missiles handy that are longer range that will do the job just as well if not better alongside other effective countermeasures. Government can't just come out and say foreign nukes are more or less useless though because that'd be the day the nuclear deterrent gets it's teeth pulled.
If someone smuggles in a bomb and detonates it without warning shelters really won't help you. Pretty good shot you're going to need to be in the shelter BEFORE the weapon detonates. Coming in afterwards just means you and the other poor contaminated bastards that think that's the right move are now in a giant microwave. Even if there's decontamination showers if they're not right at the door, someone refuses to decontaminate, or they are non-functional you're probably fricked. In this scenario you need to gtfo and decontaminate as soon as possible assuming you're still alive to do so. You might still die regardless due to already absorbed radiation, but getting out and getting clean asap might give you a bit more time by limiting further irradiation and is obviously going to be better than hotboxing with the other freshly minted glowies waiting for your skin to melt off.

That's not gonna happen for two reasons.
Reason one, and the biggest reason, is that they hate you and me. They view 99.99999% of the human race as cattle and wouldn't even bother building a communal shelter in the first place, even if their plan was to charge the hell out of you for a place inside. If they DID do that, you can rest assured that they'll have built it so cheaply that everyone inside will die, be it to the building falling in on them, the walls/door being unable to withstand a nuclear blast, or just radiation poisoning because they couldn't be bothered to make it actually protect you from the outside in some way.
Reason two, Vault Tec ran the Vault Experiments primarily for the government's benefit. They holed up on Control Station Enclave, the oil rig, and watched the experiments. The reason wasn't some sick perversion, but so they'd have a better understanding of what to do and what not to do to build a multi-generation starship to colonize another planet, assuming that nuclear war would render the Earth outright uninhabitable. Some vaults were just testing for what physical defects or cost-cutting measures to the spacecraft would do, such as testing the effects of radiation or what light level can humans reasonably work with, but many focused on the social aspects of having a bunch of people locked on a ship for multiple generations. How to dispense justice, how to ensure a stable government that remains committed to the plan, what social norms need to be upheld to maintain social order, can the crew be trusted to have ready access to weapons, etc.
Now the main reason they're not going to do that is because none of them would plan that far ahead. They don't care, it doesn't affect them so why bother? Their kids can deal with that, if they even have kids. And even if they did think that far ahead, they wouldn't bother testing anything. "Why should I test it? I'm always right"

You can make a fallout shelter in your living room. You need a really sturdy table or desk (or two). Pile 3' of dirt on top and on the sides (you can use sandbags, trash bags, cardboard boxes, or whatever you like, as long as it works). Leave a small opening so you can crawl inside. If you have an extra hour or two, tape up your windows, doors, air vents, and any cracks in your house that you can find, so that fallout particles are less likely to get inside in the first place.
Then get in there and stay in there for the first two days unless absolutely necessary. After two days, you can get out to stretch your legs and stuff, but stay inside it as much as you can stand, and do not leave the house unless absolutely necessary until the two week mark. At that point, unless you were right next to a missile field or something, it's probably safe for you to go outside and start figuring out how to survive in what's left of civilization.
What do I do if I live in a tall building.
A tall building actually provides a good deal of protection, especially if it's made out of reinforced concrete. Also, fallout tends to do what it says, and land on the ground, so being either above or below ground is preferable.
In short, if your tall building has a deep basement, that's the best place to stay, and you might need little or even no additional shielding at all. If it doesn't, then you want to be roughly equidistant from the roof and the ground, and as far inwards as you can get. That maximizes the natural shielding you get from the building itself, and minimizes what you need to add.
Your larger problem, of course, is keeping supplied with necessities and dealing with neighbors who didn't bother to prep (which is almost everybody).

Shelter inside if there is fallout, basically like a covid lockdown and recession and then rebuilding starts. Fallout from nuclear weapons is mostly very short term, its not like the material from a reactor like Chernobyl. By the time you have stayed in shelter for a month from fallout (which will not be everywhere just where the wind direction deposited it most of it has diminished in effect to the point as song as you don;t eat it you will be fine and within a year its more of less harmless. face masks and hand washing. Nukes don;t end civilisation an unles you are trying to survive a direct hit all you need to get through fallout is somewhere to shelter with thick walls, or other solid material e.g water tanks around you for a few days or weeks. After a month all that is over anyway but from memory most of the benefit is in the first two weeks. That's if fallout even comes your way

How would you build a fallout shelter in a place where you can't dig, like Florida? I'd convert a parking deck by sealing and thickening the walls.
Dig down but don't get yourself in the water table. as deep as you can. Then rammed earth walls and piled dirt roof much like a bunker would be made
Concrete has a halving thickness of ~2.5-3.5". ICF is typically built with concrete cores 6 or 8" thick but you could easily step out up to 10 or 12" with only a mild increase in cost from the extra concrete. That'll cut radiation exposure down to something like an 8th or 16th (well, not quite once you account from windows, doors and rooftops but it would still be significant). Walls would be stronger against hurricanes too.
Then dig as much of a basement as you can. Anything would help. One of my mates has a three-foot deep trench in his garage which he uses for wine storage. Something like that alone would provide a a surprising amount of protection, particularly if it's long enough for you to lie down and sleep in. Picrelated.
Finally you would pile bricks, cinderblocks and other heavy material around and over the trench to protect from skyshine and radiation from fallout settled on the roof.
Link related suggests keeping bricks etc stored in your home in the form of a bar in your basement but the same principle would work in an above ground room (albeit with less protection).
