They only get them when required, and here it was a bit too late when you already have the local friendly TZD unit in your trench.
By "body armor" I guess you mean kevlar, which is only good for stopping pistol rounds. There are heavy ass ceramic plates that can stop rifle rounds that aren't the armor penetrating variety, but nobody in that video is wearing one of those though.

M855A1 solved the fragmentation problem through hydraulic instead of yaw based initiation. It frags every time at velocities of 1900fps and up.

At close range, especially for 5.56, no the projectile doesn't just icepick through, that would be more of a worry at ranges beyond the frag threshold for a given 5.56. 55 gr M193 is going to do *very much so* just fine out of an 8, 10.5, 11.3, or 12.5 inch barrel if you're only ever specifically shooting things at around 50-75ish meters. As will frankly most serious loads of 5.56 (big caveat always obviously being on soft tissue, armor defeat is a whole other ballgame).
