You're going to need to give more information than that, anon. Where are these people? Where are they passing through? How fast are they going? Are they on foot? Is it outdoor or indoor?
Assuming its counting people going through a doorway, you would calibrate the ultrasonic for a high and low state where the low state is, after mounting the sensor, nothing is going on and the high state is the voltage with something in the door. Have a window for how long the high state can be counted as "1" person as jillions of high states will be recorded at each event.
Each high state trip increments a counter - just have an if statement check for a threshold, then send voltage to the piezo.

people usually walk in groups, most likely, a sensor like that will count a group as a single person if they stick together.

you can just wire up a switch to toggle when it rotates and simply use one of those electronic beads or counters, or even dividing chips like 4040
one way could be to use laser shining on photo resistor which is interrupted by human body, preferably legs so you dont blind anyone.

You could detect presence in a counting area then train an AI to count the number of people in that area
