    Definitely had to happen during the age of matchlocks, not just due to them being new and rules regarding safe handling hadn't been created yet but part of the process of reloading an arquebus as documented in period sources includes pulling the lit match off and holding it in your hand as you load the gun, implying someone or a lot of someones had to have accidentally lit their powder while reloading. Gonna guess the first ND was someone getting a face full of burning powder looney toons style and not accidentally blowing their or someone's head off
