It was bad fuzing, proper fuzing would have the warhead detonate while inside the bridge deck, instead it was set for penetration into a house or bunker, and detonated beneath the bridge, doing little damage beyond punching a manhole sized hole in the bridge. The russians placed a metal plate above the holes and kept driving trucks over the bridge.
No, they used a mix of instant and delayed fuzing. Some of the attacks were meant to gouge chunks off the deck while others were meant to punch the surface and detonate inside the box girder.

I don't know what's even the point of mentioning that. When someone is trying to undermine the structure of the bridge, laying a metal plate on top is going to do frick all. Not to mention the plate trick even failed a few times and trucks nearly took a plunge.
