    if I had to guess I'd say 1 out of 10 battles there's a case of friendly fire. The odds increase the more chaotic a conflict is and there are cases of simple misidentification outside of battle. Stonewall Jackson, for example, was killed by a Confederate sentry who first fired before Jackson could reply and then refused to believe the General's party when they identified them. In more recent times, the Brits still haven't forgiven us for the Blues and Royals friendly fire incident.

As for how common aside from combat mistakes the most common issue is someone being a arrogant shitbird in the barracks and getting shot from peculiar "ricochets" in actual combat situations. usually from the direction of those they abused when they were out of harms danger.       In NATO armies, you have a squad of 5-7 people and they are given information to achieve their mission. This flexibility means needing to know what they're going into, what to expect, to also be able to adapt to the unexpected. They're given maps and a battlefield situation.

      In Russia, meat is just ordered "go in this direction until you see a trench and hold it". These are simple instructions that don't require them knowing how bad it is over there and their commanders don't care, they're just meat.
