I’ve got a .410/ 45lc insert for a 26mm flare gun, and they make powder actuated crossbow attachments, so yeah it’s pretty versatile for something made to be life saving.
The videos of people getting shot with flares show them bouncing off, there is one vid of a russian getting hit with some kind of pen flare in the throat that killed him but flares are not great as weapons, especially 12 gauge orion flares.

There are a whole host of laws you may still be running afoul of, the need for rifling, the amount of explosive in projectiles, whatever nebulous nonsense the atf went after when they restricted bear bangers.

These are hail mary choices for firearms. Probably the last refuge for prohibited persons before they just decide to get a regular firearm, and definitely the first step up from having no firearms at all.
That said, there's a lot of sub cal inserts available, but you can also buy the 26.5mm to 12 gauge marine flare adapter, then add a 12 gauge to "x" insert. 9mm and .380 fit no problem. I imagine .38/.357 would as well. But my personal opinion is that the best move would be to buy two or three of the .45 colt adapters, load them with four-pellet 000 .410 shells, then lay a strip of scotch tape over them to hold them in place. At least that way, you have faster reloads.

I think you could, it wouldn't be a revolving cylinder shotgun, just an enormous .45 or .22 or whatever. You might want to roll your own inserts, make a rifling button, buy a few water keys and some epoxy, it's pretty easy to do.

Get real loser, if you’re spending that much money on the gun, you can afford the inserts, stop being poor and trying to bubba a $3k flare launcher with bondo and brakelines.
Of course it's better than nothing, a brick is better than nothing, but they bounce off people so if you're around anything that you don't want to burn down, it's not ideal.
The actual damage is pretty low, but the sudden flash and smoke and burning is shocking and so across all the dozen or so videos I've seen of people getting hit by flares, and the maybe dozen or so accounts I've read past that, it basically always goes the same way, the person shot panics and runs. Shit catching fire isn't some off chance either, every use of a flaregun for violence that I've read about that happened indoors (or on a boat) resulted in a fire, there's a fair bit of selection bias there, but what I'm saying is it's a real possibility.
A guy i know from an email list i was on just got out of jail a few months back after shooting at his brother-in-law with a flare gun with a .410 shell in it. It was some kind of domestic dispute moronation. I'm kind of annoyed about it because I'm pretty sure he got one after reading about MY flaregun with .410/.45 insert.
Better than nothing but nearly worthless. With an insert they're actually kinda useful.
I have fun reloading .45 colt with a lee loader, plus it's a single shot so I'm not tempted to blast away. .22 LR and 12 gauge flare adapter are also handy to have.
I also installed one of those pic rail rings that are for putting a microdot on a scope. My RMR is a knockoff but it's not like I'm doing anything serious with it.
I have fun with it. Trigger is absolute ass though. The inserts are a little tight so I may sand down the bases to make them fit better.
Flare guns are great for home defense. They'll burn those pesky homes to the ground.
You could shoot a 22lr out of one with the right inserts, but having only one shot makes it dubious at being a reliable self defense weapon.

Also consider that there is no size limit for flareguns or pyrotechnic launchers as long as it’s not a designated dd size like 40mm
